How we investigate
We preserve and examine the evidence, apply NDT and metallurgical reasoning to failed components, reconstruct the loading and operating conditions, and review the machine's maintenance and inspection history. The investigation follows the evidence to a root cause rather than a convenient conclusion, and it distinguishes the initiating cause from contributing factors.
